She was named Derpy Hoves by fans, and right now the name was approved even by Lauren Faust, and is now an official MLP character |D

Originally she didn't have a name. There was a pony in a crowd scene who was wall-eyed due to an animation error. Fans named her "Derpy Hooves" and started building up a bunch of fanon about her. The producers caught wind of this and turned her into an actual canon character named "Ditsy Doo" as a nod to the fans.
This is one of many reasons Lauren Faust is awesome.

Actually, not to sound smart alec, but on the mlp wiki it' said one of the animators in charge of making the misc ponies in the background made her face that way for fun since it'd be covered later. but during design the layer of ponies originally covering her face was removed, the lead director noticed her face and liked it. thats how derpy hooves came to be
