Maybe the secret is the (man)skirt itself?
With no lie, this was probably third time I tried to get this idea on paper and finally it succeeded. Somewhat, at least.
Imagine Bakura is staring at a mirror.
And no, that is not a post-sex cigarette. It's "watching-my-future-self-trying-on-my-clothes"-cigarette. ... Honestly!

The secret to look manly while wearing it is to have proven your masculinity thus the honor to wear it and NOT be called a girl. I belive the inuendo is implied here though.

I say it's the macho face scar and shortly cropped hair. The only real way to prove if any theory is correct though would be to strip both men naked and add on one piece of clothing at a time to see if any manliness if lost. Adding a scar to Bakura's face before the process and either cropping off his beautiful hair or making him tuck it in under a short wig before that test though... Then we could know for sure.
