This is pretty scary, I kinda like the blood splash above. But I think the stitches should be more 3D than just outlines, that way it would be more gruesome. Also, you should make the eyeball and the cake a bit more bloodier and some spiders can add some atmosphere to it. If you want to be scary, be the SCARIEST.
