I think it's like this:
A person with a disorder/disease, which is an "outcast" in life, travels around their imagination, therefore the title.
They make a wish, which is like things being better and stuff, they find the better world in their imagination
They're happy in that place, but at the same time, miss their home. I think that's why it's happy and sad and stuff?
