A game over graphic from that I thought I'd finally post here
It is one of the rarer game overs since you can currently only trigger it in one place one time.

I went back to what I think you mean by "water cave" which is the scene where you drink the water and you see the magic door appear. I didn't find any way to trigger this game over there. Did you mean some other cave?
