Just a steady hand, I guess. The version of Photoshop I use doesn't have a pen tool, so it's all by hand.

Mine doesn't either. When I do one stroke with the paint brush though it looks really faded so I have to go over it a lot of times.

I make sure to use a hard brush at 100% opacity. Some of the softer brushes make the lines look too blurry.
