I think it's different realities because Red knows Professor Oak and so does Ash. Red meets Misty and Brock but doesn't really become friends with them like Ash does. Red and Blue are rivals like Ash and Gary are rivals. It seems like they exist in different realities like you said.

They're the same person in different realities. Ash is heavily based on Red who is the protagonist of the first game. Even Pokemon Adventures Red is based of of game Red. They're counterparts.
