In the panel when Byakuya is thinking that Ulqui missed, there's this tiny arrow above that in the center of the speed lines. Is that just something else and I'm mistaking it for an arrow?

Yeah, that's a spec in the distance that Ulquiorra noticed...it's Ishida and Orihime coming down from the dome. That's why Ulquiorra missed with that strike.
