There's actually a few deaths I missed the first time around:
In the room with the Mona Lisa paintings, if you answer the riddle questions wrong then read the note on the wall, it will say "Are you answering wrong on purpose?" Say yes and get crushed.
After parting ways with the frog, go back in the room where you first found him. There's a startling surprise in store.
If you don't run back up the ladder in the fireplace fast enough, you'll get knifed.

To get the third ending, never talk to the black cat even from outside the house. Ignore it, that means, you should never have a saved file to achieve the third one.
