I got some ideas
Spontaneous Combustion (passive)
Icon: a skull on fire (isaac's head is on fire)
Everytime you get hit, you explode.
Swinging Toy (familiar)
Icon: a hat with a solid object connected to a string (Isaac wears the hat)
The faster you go, the faster this spinning attack fly goes.
Special Eyes (passive)
Icon: A blue eye (when used, Isaac's eye become blue)
When you use this item, it gives you 2 random tear effects (kinda like 3 dollar bill)
Takes 3 rooms to charge
Dysentery (passive)
Icon: Lots of poop
This item covers enemies in poop when you use it.
Takes 2 rooms to charge
XXII - The Gambler (tarot card)
This card teleports you to the Arcade (if there is any)

Item icon: a dollar bill on a hook
Function:launches a laser simular to Shoop da whoop! but dosent require a charge, and is stronger, and makes enemies slower, and the Dollar bill is more possible to find in Vending Machines, however, Its extremely rare, and using it makes more enemies powerful and more likely to be Champion Variants.
