First, brilliant work!
With that said;
Too much work in the eyes, tooth on lower right is popping something, face is treated nicely but forehead could be smoothed out to match, The hands are way too much. Graphically, competing with the head/face.
It feels odd to critique the work of an artist more skilled than I.

My two cents. Basically flawless except that it's too noisy. Yes, I know it's intentional but it's a bit too heavy. The beauty of this image is it's simplicity, the noise messes that up and takes away from the sharpness of the facial features.
