It depends, background ponies are put in there by the animators, nopony of them are related. Only exceptions:
Daisy, Rose, and Lily are sisters and Goldengrape is Daisy s' boyfriend\fianceè.
Quite a few BG ponies are part of the Apple Family, with Sweetie "Bon Bon" Drops, Caramel, and\or Mr. Greenhooves being either family friends, or very distantly related. Caramel is also Sassaflash s' boyfriend, and has many "versions" of him that may be his relatives.
Thunderlane and Rumble are brothers, and Flitter and Cloudchaser are sisters.
Lyra and Bon Bon are seemingly together.
But other than that, family relationships are a bit wonky. Even Dinky has seemingly two pairs of parents, one is Carrot Top and Written Script, but she is also the kid of Cloud Kicker and Doctor Whooves. And she was seen in a sister-racing competition with Amethyst Star.

Huh. I never thought of the flower trio as sisters, more as close friends who split a business venture.
And yeah I get that they're just put there randomly. It's just fun to come up with family trees

I once saw some identity cards saying all of them had the last name "Flowers".
But last names can actually be changed & some pony families do not seem to actually have last names, so...
