Description
Based on 2 page 1896 registration forms from St Laviours Union Parish. I made it 3 for ease of printing, and changed it to include a letter from the doctor.
Poor Miss Leading, she didn’t start out insane but she might have listened in on mob plans. Really once a person gets locked up in Arkham they really are forgotten about, ne? Unless they escape.Download for the blank forms and an example letter.
And you so know that Nolanverse Crane would use a stamp. Its so not legal but he’d so do it. This is also a present for since I keep forgetting to review her work but love it so much.

The font of the forms themselves? Um. I don't remember but it looks like Times New Roman.

I just typed the text then used the preset shapes tool (I used paintshop pro for this but I think photoshop has a similar tool), set it to rounded rectangle and a long dash/short dash setting and then surrounded the text. And to make it look a little more human I put is slightly at an angle.

You should be able to download the zip file, use an archive program like winzip or winace to extract the files and then edit them in whatever picture program you like (photoshop, paintshop, even MS Paint).
If something is wrong with the files let me know so I can fix it.
