Here is the download for the MMD file of a 3D Chakram for Axel from Kingdom Hearts 358/2 Days.
There are a few sliders in case you wish to animate the weapon.
Modelled by me on blender and textured using photoshop using the in game screenshots as reference.

I think that's the idea the designers were going for. Not sure if you've played the original Final Fantasy VII, but Yuffie has an Ultimate Shuriken weapon named the conformer. I think this weapon was a reference/homage to that
