Yes, 5 of these forms appear in one episode - ring any bells? This was an episode where Shingo, the guy who Ankh possessed, is trying to be a replacement for Ankh while he's out of the plot, and there's a scene where he practices giving Eiji medals to use in battle. So, it was a montage of different combos being tried out. Very fun moment.
That point when Shingo was in the show, it was actually where I caught up on OOO while binge-watching it, and was where I begin watching Rider as it aired, so it's oddly nostalgic working with this set of combos.
Oh also, we're done with the random combos that appear in the show! But there's still some more.
