Stealth in D&D is a very odd mechanic. Its one of those things that just works, and if you try to fix it only makes it worse.
I use the specialist skills from Lamentation of the Flame Princess rules. Basically a x/6 roll. Every level you get 2 points to add to any of the kills. I think it works better then the d100 system from the original.

In this case Ally failed her stealth check when opening the door, and when sneaking into the room. Luckily the inhabitants of Wonderland are idiots.

The first of many many times I forgot to include raven.
