Not super happy with how this turned out, but oh well.

I'd dare to say that these two having ASPD is as certain as Pyro having schizophrenia. They're both arrogant, sadistic, violent criminals that completely lack remorse for their actions. The only time I've seen either of them show legitimate emotion is when Spy mourned Scout in the last comic. He even seemed angry, embarrassed by it.

I'd say Spy is a psychopath and Medic is a sociopath. Psychopaths are cold, calculating, controlling, other descriptive words that start with c. They tend to live rich, powerful lives, since they have no problem with manipulating and pushing people out of the way. And look at Spy, with his sports car and silk suits.
Sociopaths, on the other hand, are impulsive (as in switching from one team to another, then going back and expecting everyone to be okay with that) and haphazard (as in not using any sedation or pain killers during open heart surgery), and usually have to rely on criminal activity to sustain themselves.
