After more then enough failed routes and plans trying to get Sans on his side, he realises this way won't work, so decides to take it upon himself to take down the human, returning to the underground, beginning by extracting DT from the amalgams, injecting it into himself to stay stable longer, of course this is not a long term plan, and he needs to use his abilities sparingly and inject new DT into himself often.
After having taken and extracted all the DT he could, he destroys the DT extractor to stop anyones attempt to stop him, beginning the killing of the monsters, they're going to be erased anyway so why bother keeping them alive now?

Geno is calm and calculated, using his resources sparingly and making sure that there are no ways for his plan to fail. Killing anyone in his way bar Papyrus who attempts to keep locked away. He wears an old lab coat and a scrap of the DT extractor as clothes along with a visor to hide his horribly melted eye to attempt to not raise suspicion.

Sans was one of the first to go as Geno knew he'd be a big problem if left alive, his soul lives on in him as Geno was able to absorb part of it, Sans begging Geno to stop or reconsider, but it's too late for that now.

Chara and Frisk have re-united as while they hate each other, neither want Geno's plan to succeed, Frisk taking the back seat to try and give info on Geno as the narrator, while Chara does as they do and kills anyone they can.

Papyrus also helps with the humans, unaware of what's happening, but wanting to get through to the "MONOCHROME COPY OF MY BROTHER", with Frisk and Chara keeping him around due to Geno refusing to hurt him, though Chara now has to hide their murders from him.
