I actually believe that’s orihime behind her. You see that blank white space? It’s the bottom roll of Orihime’s belly. Zoom out and it’s basically a mole hill of Rukia next to the mountain of Orihime. The same goes for the twin peaks of Rangiku and Hallibel behind her
