Naw, they're all there. It's just that the middle finger is sticking out making the index finger look like the thumb when it's really just behind it.
The thumb is probably hidden behind the hand and the other fingers.

The thing is in this case it's not possible for the index finger to be behind her other fingers, the way it's drawn makes it connected to the middle finger itself. Leaving an impossible gap between the middle and ringfinger.
