I think that he fell in the truck, not on the ground! That's why Sherlock told John to not move and watch him.
He jumped and John was only able to see him fall and then the cyclist pushed him...

I couldn't come up with any theories at all... But I do now that you can slow your pulse so it seems stopped, if you have a tennis ball clenched in your elbow. And we did see Sherlock playing with a tennis ball earlier. Which would explain Why John couldn't feel his pulse.
