Actually, that's always been a question I've had about this world. How do you determine the benders? Does it manifest by a certain age? Do you just have them do the forms repeatedly until fire, earth or water starts moving? Do the really desperate parents keep have their kids do this for hours on end, every day?

It's determined by making a call to the Bending Ability Center in the Spirit World. Families in ATLA and TLOK are usually small because of the insane cost of making this trans-dimensional phone call, not because of anything else.

When young a bender is unable to control their bending. When a young bender is brought next the their supposed element, it will react. This is seen in the last airbender movie.
