Sam doesn´t see Dean (angel) who´s there to save him
and the face in the background is (or should be) Lucifers

Ahh, okay! I just wanted to make sure. I kinda figured that was Lucifer's face in the flames and then Dean behind Sam as an angel. I have no doubt Lucifer did this kind of thing to Sam. No doubt at all. You did a great job here!
