I am modifying this for an ampharos hat. How do you get the ears to stand up?

How would you sew them onto the hat? Just stuff them and them sew them, or leave them empty and sew them?

Sew enough of the ears to be able to fit the stuffing, and then flip the ears inside-out. Then stuff the ears with just enough stuffing so that the ear is neither floppy nor overstuffed. Finish sewing the ears shut with a thread and needle (whipstitch)-- I tend to fold the two flaps inward to match with the rest of the inside-out seams. Finally, use a whipstitch to attach the ears to the hat-- be really thourough with this step because the ears can be torn off if you aren't.
There's a little bit on how to quickly whipstitch on my tutorial, and there are tons of websites that teach it as well.

are these to scale if you scale it to an A4 piece of paper?

I'm not sure about A4, but if you print them to fit on an 8 1/2 X 11'' paper, it should be very similar to the size I used. ^__^
