You captured everything from the fear, to the changing to pain she loses her body. from the fussing go her arms and tail. I hope you will do more. Her face changing. Maybe add a part of her opening her eyes and seeing what she has become.
What I see looks like it comes from a movie. You know on some of the victims kept their hair or more of their face. I wouldn't mind seeing you add more to this another part which show her have the contract going at from Ursula.
Maybe you could do the part when she is turned into the human. Say when Ursula throws ingredients into cauldron.

Actually, I was thinking about the fact that Ursula's contracts like their curses are negotiable and transferable.
However the goal of Ursula not was transform Ariel into a Polyp but seize the trident of Triton, so anyway still not a good end for her.
