Well, if you want get four of the doctor medals that require you to XS everything in surgery, first response, orthopedics, and endoscopy, then yes.
Sadly, I'm not sure how many doctor medals you need to unlock Albert's line. For the other medals though, you can leave it at Intern or Resident (to my knowledge at least).

Yup. I chickened out of getting those dang doctor medals that involve no stabilizers and ended up running away to Intern. O.O Intern is effing EASY. I can finish a majority of the operations without even using stabilizer...........
But yeah, you have to XS on Specialist mode. I've only gotten two of them (Frozen In Time and Foul Mood) and I'm not looking forwards to Love in the Ground.... check the XS guide here: [link]
