Well it's a redesign of Foxy, and the kids just "can't keep their hands to themselves", and basically they staff had to put it back together at the end of every shift and so they just left it as a "Take apart and put back together" attraction.
I think it says that in the 2nd night phone call in FNAF 2

Yeah I mean I know that, but why would there be two heads anyways, unless the kids I guess grabbed it and shoved a head on it

They probably added more parts to the collection over time.
I mean the thing doesn't even have a torso. It's literally all limbs.
