Uh, usually it's from things around me that I hear (I was watching That 70's Show when I did this, so if someone says something like "I'm nervous" then maybe I'll like the word nervous and work it into a title).
It's like those psychology tests where you think of a word really quickly associated with an image. Kinda fun.
