So, I changed the ending a little bit and added an additional phase 4.

-Story-

“In the last corridor Frisk feels Sans' presence, sans is standing at the end of the hall with a skewered papyrus in his hand, sans starts twitching and launches himself at Frisk, dropping papyrus's head and starts sending attack after attack, Frisk would sit in the save screen trying to think of a plan to kill sans.”
So basically Paths Of Madness starts with this. Frisk being “stuck” in the save screen thinking of a plan to kill sans. Frisk finally thought of a good plan, and it worked. Sans was killed and the human reseted in hopes of making everything normal again. But Sans remained the same because of his artificial determination, the wound that the human caused heeled but the damage on his jacket was still there. Sans filled with anger tried to relief himself by killing monsters. In waterfall he found a mysterious door that he never saw before, he entered and saw someone who he thought he will never see again… Gaster (I know big reveal) They talked, and they agreed on that when the human is weak Gaster will come to help delete the human. But Gaster had a different plan he won’t just delete the human he will delete Sans too.
After a little while the human arrived at the last corridor. It went the same as last time, but Sans was prepared. He blocked or dodged every attack but after some time the human was able to lay a hit on him. Sans collapsed on the ground. (He didn’t die because he gained LV after the human reseted.) The human tried to hit him again, but Sans blocked it. He got up, told the human that they have to be stronger to kill him. He grabbed Papyrus’s head and created a large sword and thus the fight started. The more he used his powers the more his body melts and the more it withers. At the end of his final attack Sans faints. Suddenly the human got trapped in blue bones. Gaster appeared and told the human that he will delete both the human and Sans and reset back before he injected Sans with determination. (Btw Gaster caused Sans to faint.) Gaster tried to delete Sans, but he was unable to. Deleting Sans only made him stronger his body became invincible. He got up and deleted Gaster. Then Sans started to fight the human, but the human was unable to kill him. Sans eventually stopped and deleted the human and the game.
In the end Sans was left “broken”. His body literally became invisible, a ghost of sorts. He began to rebuild himself with his new powers…

-Appearance-

Phase 1: He looks the same as regular Insanity Sans but has a hole in his jacket.

Phase 2: He uses a sword that he made using Papyrus’s head. His skull is melting, and his body became to wither.

Phase 3: His body became invincible and melts even more. He is constantly glitching and he has error messages around himself.

Phase 4: He rebuilt his body and became an enormous, unstoppable beast. (This phase only exists in case of a LB!Mirrored Insanity)
