By a reason not totally understable to me, I see this photo not as a realistic image, even as metaphore. The woman represents the angsty who habits to human being...this angsty that doesnt "politically correct" to exhibit in public, neither front the mirror because we know that our body is the favourite place of the demons of this time.
Angsty -the feeling more appreciated in the imaged- is equal to shame...and the shame could be hide in a dark room....and the dark room -our interior- is not precisely a clean please: Our interior has damp walls about to collapse...has a little window that looks as a deathtrap. Impossible to run away. We are our own trap...Our own grave....and the metallic gray of the tone reflex this quiet desesperation. Even the woman seems scream, I only listen....silence. Silence. Silence. But a silence who cuts....a silence that hurts deeply. I have cried to this amazing photo. Is so, so far of the beauty, and, at same time, the reflex of the genuine beauty of a tortured human being.

This critique is great! I am very proud of your ideas, even if you have English trouble.

Art expresses the soul. This art expresses the soul of anguish, a "tortured human being", trapped in our own interior, a dark room that will almost collapse. The soul wants to scream but the scream is silence, and the silence is more deadly.

Without more context it's hard to glean details about what is happening to this woman. The title gives a clue.
Could be any of a number of forms of mental illness:
- Severe Depression, Bi-polar disorder;
- Schizo-Affective (she might be covering hear ears to block out voices);
- Substance abuse;
Could be suffering an emotional breakdown, suffering from the emotional shock from the sudden death of a loved one.
Could be unrelenting physical pain.
But the image does capture a person in a state of extreme suffering, for whatever the reason, maybe the context is unimportant and the only thing meant to be shown is just a state of human suffering.
It's a powerful image no matter what the context.
