Name : (the name for your OC, just use a original or unique name to fit your OC.)

SOUL form : (only required for the humans or half humans with a magical form outside their
physical bodies, if your OC's a monster, cross out the "SOUL Form" name and put "ALTERNATE
Design" instead.

Age : (the current age your OC, both human and monster age are allowed.)

Likes : (what your OC like.)

Dislikes : (what your OC don't like.)

Species : (the group your OCs in, humans or monsters, hybrids are allowed as well, not like
the amalgamates though.)

ATK and DEF : (the states of attack points and defence points your OC currently has. HP as well
for health points.)

Skills : (at least fit two talents your OC has, list the others in your description.)

Extras : (list these in the description.)

Friends : (list who your OC is friends with, both Undertale characters and other OCs and who they are to your OC)

Family : (list all possible family members your OC has, doesn't have to be biological, foster family members are fine too.)

Job : (don't use this if your OC is too young for a job, at least if they have the right age they could have a job, but if they
don't, just put them as unemployed)

Attack abilities : (only if your character knows any magic abilities)

Weapons : (any simple or magic weapons your character can use is fine)
