While Ruby and Brendan have the exact same appearance, they are completely different. Ruby is basically the manga equivalent of Brendan... except you have to remember that everything from the storyline to the character personalities is completely different in the manga.

Ruby is a very feminine boy who competes as a coordinator (basically a groomer) as compared to the very masculine Sapphire (based on May) who competes as a battle trainer. To my knowledge, it is reversed for Brendan and May.

Ruby is also good at making clothes... as he apparently made the outfits that both he and Sapphire wear.
