I always felt that the Sniper and Spy would make ideal classes that serve a leadership role for either team. Because the role of a leader is generally a "supportive" one and these guys are classified as "support" classes. So why not?
As for medics? They could be second in command.

Honestly, that's the way it is... The chain of command is
-Spy
-Engineer - Medic
-Scout -Soldier -Pyro -Demoman
And then Heavy and Sniper are not in the chain of command because they don't care.
You can see it in the comics and the Valve videos. The Spy is the one who chats the higher ups and makes plans, the engineer and the Medic are both smart but they don't work as leaders (The Medic is there because his team is his guinea pigs). Then you got the Scout, Soldier, Pyro and Demoman who all respond to the chain of command but aren't in any position to do anything besides experiencing psychosis.
