Equestria and the Dragon Lands don't have the same pathogens. Spike's three-month stay in the Dragon Lands made it only a matter of time before he would contract something his body wasn't used to. Thankfully, the virus he caught isn't nearly as heatproof as a dragon, so his hosts prescribed a tried and true treatment: "cooking it out". Spike was cured in no time and could kick himself for not thinking of this method sooner. The next time he caught a bad cold back in Equestria, he prompty soaked in boiling water and saved himself multiple days of misery.
