A question: how are the characters commissioned to you? Do you get a description? Of the overall character, or even of each form?

I get a character description, how detailed that description may be is quite varied. Sometimes I get a lot of freedom and sometimes close to none at all. I'm also pretty sure 50% of UR characters are parodies of existing material (ie Machete & Hulk Hogan in this picture)
