I've always thought that it was in the support structure for the nacelles - its the only place that really makes sense to me, plus it could allow access to nacelle internals.

Maybe in the "spine" running along the top? The warp core is up there IIRC, and it would be a way to get to the aft end if the main central module section is taken up by things without habitable spaces or left empty for one leg of a trip. Or just in the versions that don't seem to have a door at the back end of the cockpit block, without having to use the transporter to get there.
