Would you believe that out of everything in the original "bell the cat" picture, that silly little detail is the thing we spent the most amount of effort trying to get right?

Why don't people make more custom backgrounds in Kisekae?

Probably because it's actually deceptively hard. Admittedly this one was pretty simple in it's construction, but sometimes what appears to be a simple prop can actually be composed of a dozen or more separate elements that all have to be carefully manipulated and arranged independently. While it's certainly possible to create beautiful custom backgrounds using in-game elements, especially with the improvements that have been made to the program as time has gone on, it's not what the program was made for.
