This is if Runa never recovered from the death of her brother and she became unhinged. This is the 'Mad Wolf Queen' Runa Stone-Wolf. She has been seen talking to herself but she tells everyone she was talking to her brother as she can see and hear him as if he never left and kills anyone who questions her mental state or try and convince her Tayio is gone. This explains why her claws and hands are colored red over time and noone in the village of Stone Ridge questions their ruler.
