As far as I know Tamoe found it in a second hand book shop and then modified it with FIMO-parts. But check her gallery, I think she has some photos of when she made the book:

Ta-moe made this book. She took an old book she found in a second-hand book shop and then she did the buckles out of papier mache. You can find a more detailed photo of it in her gallery:
