but i think it be hot if you add a variant of her taped gagged and otm gagged because mask or muzzle gags aren't as sexy and sensual imho as a regular cleave gag or cloth gag

Well, that's your opinion. Personally, especially given the institutional setting, I think that a purpose-built muzzle is the most appropriate gag for this situation.
And to me, the heavy-duty-ness and the context are what makes it sexy.
(I love Ball gags, but I think they're more appropriate in a dungeon than an asylum.)
