If you think about it, Buffy has died thrice.
At least I count that time when she died in one of the last episodes from season 6, the one Dark Willow removes the bullet from Buffy's heart (Warren shot Buffy and killed Tara) as death. So, for me, Buffy has died three times, not only two.

I don't know if she has died or not in the comic books, but in the tv show I count three deaths. The Master killed her on Season 1, then Glory killed her on Season 5 and Warren killed her in Season 6.

I thought for sure it was four, but you are correct on it being three. But only twice did a new slayer come into play. I think that a third replacement slayer didn't cause Faith was still around. But, yes, 3 times.

Yeah, due to her first death Kendra became a slayer. The Slayer line was passed to Kendra, it didn't belong to Buffy anymore. Proof is, when Buffy died the second and third time, no Slayer was called because Faith was alive (and Faith turned into a Slayer because Kendra died in Season 2).
Buffy was actually wrong when she was talking to Giles in Season 7. They were talking that the first was after the potentials "and then Faith, and then me. No more slayers" she said something like that. Actually, Faith should be last, since Buffy is not part of the Slayer line anymore.
