Well, I was really obseessed with his character a while ago, so i studied him closely, and made certain conclusions.
Based off his model and first concept art, you can see that his head is not exactly a skull, but rather a slightly scull-resembling mask, and knowing how shadow isles residents can be both ethereal and phisical, i thought that it is quite possible for his mask to have expressions. You can see it well in that Dante guy's videos on youtube for example.

He was my main for a really long time in league, I really like his character, and I even wanted to make some comic with him, but couldnt come up with proper script. So now I have a ton of sketches and unfinished things, but nothing serious.

oh man yes, i love Thresh as well, but i'm very reluctant to actually play him because my confidence immediately drops as soon as i miss a hook or something x'D not a good mind-set, but eh, it's difficult to shrug it off.
i would really love to see a comic featuring him. have you seen his new lore? you could draw it as a comic. or just Hecarim and him bullshitting around idk xD

the new lores for Heca, Thresh, Morde etc were actually released a while ago with the Harrowing event i believe.
