This is an alternate universe that starts to deviate from canon following "The Cissy" and would have taken place immediately following, or during, this episode. When Stan feels confused about what he is, he thinks very hard about life and the people around him. Soon, he discovers that he doesn't feel right in his own body. It is then that he realizes that he is a female and that he is attracted to guys.

He would break-up with Wendy, but would try to remain friends with her, as he still loves her and remembers the way she makes him feel. Then, he would kiss her, for the first time, without vomiting.

His mother accepts him, fully, and embraces the decision, without hesitation. Randy, however, feels as though this isn't really how Stan feels and that his confession has caused Stan to become confused. After a few discussions, and a little bit more time for both of them to think, he realizes that Stan is old enough to know what he is and decides that letting Stan be who he is is the right choice. Shelly still hates him, but after seeing what he looked like as a girl, she made a quick comment that she liked his hair.

Stan chose the name Stephanie. This is because Stephanie still contains the letters to the name Stan. This signifies that he is still Stan on the inside, since his old name is hidden in his new name. He has updated, but hasn't changed.

Cartman reacted just as you would have guessed. He made fun of her and laughed, immediately. He doesn't really care enough to stop hanging out with her, but finds it funny enough to make fun of her, forever. The change didn't affect Kenny and he acted no different towards her. Kyle found everything to be very awkward between them, though. After trying to hang out with her a few times, Kyle told her that he needed space. But, they quickly reconciled and began anew. This eventually blossomed into an even closer friendship and, soon after that, a relationship.

Also, as for Stan's physical transformation, he got surgery to become an official woman and wore extensions until he was able to grow out his hair. He didn't do anything to his chest until he got much older and needed female-sized breasts.

P.S. Cartman's super jealous because he likes Kyle, too. I just had to throw that in there.
