Overall, great drawing... Goog shading, loads of detail, although he does look much more human-like than in the game. I guess that the point, huh? Il like the backround and the chromatical purple colours blurred to make The Heretic stand out. Shame there is no pentagram underneath him instead of the cross, as that is how he is summoned in the game by the half-heads. Maybe you should have left some space between the ribs and a bit more shading on the hood.
Indeed, if I found this spirit doing his bussines in my toilet I would not atempt to anger him,because if I did... I would probably get sent to the Dark Dimmension to suffer and cry, just like Isaac, surrounded by half-heads. Great job!

First of all, thank you so much for the critique! It's really well done and certainly looks sincere.
As you may noticed, ribs are quite my weakness overall, and thanks for pointing out also the lack of shading on the hood, i'll be totally working on that in the next drawing.
