ONLY two have the name doctor, and those would be the doctor [from doctor who and Voyager]

Doctor is also a profession, so "I'm the Doctor" is as valid as "I'm A Doctor"

but using the instead of a refers to the fact that you're the only one or that your name is the doctor, not that it's your profession. Bones is A doctor, but Bones is not THE doctor, the doctor is either the doctor from voyager, who's name is THE DOCTOR, or Doctor Who, who's name is also THE DOCTOR.
