It's pretty much obvious that unlike God and Lucifer, Death well known to being truly and purely neutral (being omnineutral to be more precise). And I can assume that Macaria is also omnineutral.

Both faces are neutral in terms of carrying out their general duties, but I will say that the mannerΒ of carrying out their duties is affected by a sort of eternal moral justice. For instance, if a colt or filly died young, there would be no question about death taking them to the other side. There would be no begging or pleading for "second chances" or continuing to live, because their time is decreed. However, it would almost certainly be Macaria coming to collect a young pony (since fillies and colts are almost always too young to be properly "evil" ) . Macaria, being the more merciful side of death, would do everything in her power to comfort the colt or filly about passing on. She does the same for older ponies, but she's particularly tender toward those who die young.Β
On the flip side, she does nothing as Hecate to comfort or make lighter the terror of death. She knows which form she takes, and she knows "which ponies have been good, and which ponies have been bad", as it were. Nopony can convince her to be any less terrifying than she's meant to be.
I haven't decided yet if she does all this and still remains true neutral, because it's technically her jobΒ when she appears as Macaria to be the merciful side of death or, conversely as Hecate, to appear as frightening as possible. I'm not sure if she can form actual bonds- haven't decided. She may be true omnineutral until the end of time, when she'll finally be allowed to be her true self and form bonds- like there's a block around her keeping her from earthly feelings and such. I'll have to think inordinately about it. In any case, there's absolutely no wheedling or sobbing her into not doing her job, because she's gonna do it.

Normally I leave death and the afterlife a mystery in my works, but I may make an exception.
I do have one observation, though. You used the design of the Windigos for the good goddess of death, however the Windigos were malevolent. Further, their design, while equine, is kind of "off" from the perspective a pony, and would fall into the Uncanny Valley for ponies. Not necessarily a bad thing, but she could certainly be off-putting to some ponies.
Of course, angels could be rather... strange in Judeo-Christian lore as well. There's a reason they say "fear not!" when approaching a human, after all.

I had thought about that possibility of her being off-putting, as death might be. Β I almost went with a more Celestia-like design for the body, but I wanted Macaria to stand out as something even above the alicorn princesses. Β In the end, the windigo design worked in terms of making her look taller and more graceful and obviously not another pony. Β Her form is familiar, but still somehow alien. Β I was kind of thinking of the way the characters in the Lord of the Rings react to seeing elves (particularly Galadriel and Arwen) . Β There's a certain awe in the sheer beauty of the elves vs. everyone else, and that's kind of what I was thinking of here. Β I wouldn't be surprised if some ponies were still afraid, though. Β (And yeah, Judeo-Christian angels....eye-shriveling brightness and unspeakable power. Β "Fear not", indeed! )

My idea for her is that she's death for that whole world, so she appears in the guise of whatever species she's coming for (pony, zebra, gryphon, etc)
