You have found me. I'm glad you like it! Here's what I did:
I used DAZ Studio and The Kids 4 model. First, I found some freebies that suited the image and retextured them to look like BEN'S tunic. As you can see, I sort of failed with the hat (LOOKIT THAT GODAWFUL SEAM). I'm pretty sure I used 3Dream's Helios hair for this render, because I was far too lazy to paint it and never expected this image to be popular.
then I set up the scene, morphed the model to look like BEN, posed it, then added some (basic) creepy lights, and rendered. Took about 3 hours, give or take.
When my render was done I exported it as a .png and took it into photoshop for postwork. I tweaked the skin texture, tweaked the lighting and added the blood. Took about half an hour.
Then I added the plain black background and the text, which was definitely the easiest part.
Looking back, I think I should have left out the blood, because bleeding eyes are kind of cliche, and honestly, BEN is creepy enough without adding bleeding eyes.

except the bleeding eyes might've turned out to be the best part. made it more creepy on top of its already creepiness. and since you've added it ive noticed it inspired many artists who have drawn ben since to include bleeding eyes. you should be proud that that is now a part of the ben character, and it happens to have been your contribution. kudos!

I can't believe after all this time here, I have only just gotten to one of the most intricate and eerie creepypastas of all, not to mention the second one I discovered (the first was Lost Silver). Yeah, this one was very entertaining, and gave me quite a rush... but that's for another time.
I never finished playing Majora's Mask (having to repeatedly go back to the beginning and play that damn Deku Scrub arc annoyed me to no end), but I know enough of it to understand what was happening, and wow, what a wonderful job you have done here. The pitch black background gives me the impression of infinity, and the bloodstains under the empty, staring eyes? Absolutely fabulous, not to mention creepy. I also like how you've managed to add a touch of genuine childishness to the facial features and grin, making the statue look even more like a genuine child. That only adds to the effect even more. Everything is just well done here, adding up to one scary pic. I've always wondered if the statue that keeps appearing was supposed to be Ben at all, or maybe just some sort of psychopomp that orchestrates the terrible fates that occur to the player. That's just my opinion, though.
