If the cause of his transformation the same as how he died in 4, would it count as STD?

If by that you mean that Lord Saddler caused it then yes. He turned him into verdugo so he couldn't help Leon (the latter would just attack him as an another monstrosity) and also die at the hands of his ally. Luis keeps his human mind after change so this punishment is even worser.
P.S. No, it wouldn't count as STD.
