How do you use the itunes included in the skin ?

Just run ragna_itunes.ini if you want to use the itunes one, otherwise use the ragna_winamp.ini

Does iTunes have to be open for it to play ?

Yep iTunes needs to be open, the rainmeter is just there so that you can play/pause/stop the current track/song while your iTunes is minimized.
