I so want to use it, but there are a lot of data missing from it... like, jutsu types, family, weapons, just to mention the most important.
When I made mine (only a list, not a picture like this, though), I looked at various characters' profiles on Narutopedia (XD), that table has everything that's important. ^^
But this still looks very nice, only I have a lot of things about my character that wouldn't fit into the space provided. ^^"

if you look at my example you'll see there is room provided at the bottom for including any extra information. Do remember that a profile isn't supposed to include the entire life story of the character, thats what fanfiction is for. A profile is merely a snapshot of the most significant facts about the character. Not all characters have families or use weapons, so it's pointless to include that in a template, but you can obviously include it in the bio section at the bottom.

I didn't mean it like writing everything down... I thought it'd be important to mention the jutsu types the character uses, especially if they are unique. I wasn't going to include the entire life story either, just mentioning the family members' names, if they have any, etc.
I wasn't saying your template was bad, in fact, I really like it, I was just basing my own OC profile on the box-sort of template that's used at wikipedia/wikia. (Basically, I used Itachi's profile.
